I made this in php5 a while ago (pretty sure it works with php7 but haven't tested yet).
I recently placed it on github at http://ift.tt/1UCz9mL
It's basically an error page. It allows for a logo if you pass the correct parameters to it. It also allows for the goof message to be disabled. You can also set the title for the error page to display (again is a parameter).
I will soon be adding how to use this with Nginx, apache, etc.
But for now its in development. If you like...
