Need help to adjust Adsense account

I want fix up my crawler error to add "Crawler Access" with my adsense account. Pls see the pic of Crawler access" section, click Add a login.

In parameters section, there are two rows with two blank space for insert information.

In 1st row I insert my username but what will be next right blank area.

Same way I put my password of my website login, but what will be next blank space.

Please help me.
